LIFEBRIDGE CONTINUUM OF CARE

Starting as a single group home provider in 1996, Intercept Health's primary goal was to redefine the group home experience. As the needs of families and communities changed, Intercept Health responded with the development of the LifeBridge Continuum of Care. Intercept Health's LifeBridge continues to evolve through collaboration with community partners to create innovative services.

 

The LifeBridge offers a continuum of effective services for individuals and their families who are experiencing functional impairments in the home, school or community. The LifeBridge provides varying levels of services to children, adolescents, adults and families. Whether used individually or in a combination as wrap-around services, the LifeBridge provides the advantage of flexibility to ensure continuity of care.
